would be helpful if you filled out the appropriate forms and either
mailed them to us before your appointment or else brought them with
you. If you can print them out, then please see the online forms section.
Or you can call (206) 623- 7591 and we will send them to you.
There are more forms in the "Afraid of the Dentist" section if you
feel you have had a bad experience with a previous dentist.
We would like you to be as involved
in the exam as you are able so that any discovered problems are
real for you. . As problems are discovered, they are immediately
shared with you and we will ask your opinion of how you want to
deal with them. If there is an area of your mouth that you need
to discuss with us, we can project it onto the computer monitors
with our intraoral camera. That way you can see the area for yourself,
thirty times life-size.
New radiographs will be taken based upon your dental
history and present condition. If we have your previous radiographs
we would compare them to your new films in order to tell whether
your current dental condition is stable. If your previous radiographs
are available and current, no new ones will be taken.
